Abstract sketch peacock made by Ms Gloria Poole, RN, artist of / in Missouri on 31 May 2015

This is a sketch I made today after lunch and it is ink on paper that is 11 x 14 inches in my sketch book. As usual I signed this when I completed it, photographed it , uploaded it to this blog of mine. This is about the tenth time I have made a sketch of peacocks. I posted most of those to other blogs of mine and you can probably find them by Googling my name such as "Gloria Poole + bird sketches" or "Gloria Poole + peacock sketches". Also on my https://gloriapoole.blogspot.com blog I posted links to other sites that I put my art on, so you can view all of those and scroll the archives when you have time. Also, you can follow me on twitter to be alerted when I post art I make or words I write or photos I photograph on this twitter accounts of mine: @gloriapoole; @gloria_poole; @ProlifeNurse; @tweetie0817 , @Tartan_Bliss and @personhood1. You can see more of the art I create in previous posts via links, and on the post and about me page on my https://gloriapoole.blogspot.com. I just didn't want to research medical info today so decided to update this with art I made. I have to remind public occasionally I am same single-again [since Oct 2007] white woman who also photographs extensively and makes art almost every day. And occasionally, I write on nursing or medical issues to educate the public. You can read some of those also on this of mine: https://twitter.com/ProlifeNurse.

This is my selfie of me Gloria Poole in spring 2014 sitting on my sofa in Missouri; but it is my most recent selfie. I am still putting my hair in a lop-sided pony tail however and look the same.

For the record, I never worked with or for my 2nd exhusband [with initials DBP in Colorado] in any capacity and he never worked with or for me in any capacity. It is necessary to write that because he has NO authority to sign my name to anything nor to hijack any account of mine, nor to represent himself as any sort of agent of mine. He injured me several times, and he tried to kill me four times and said he intended to annihilate me from life and the web. DBP by any name is not a business partner of mine and never was, nor is he a courier, nor associate, nor employee, nor assistant ,nor boss, nor supervisor, nor agent, nor "co-worker" of mine AND NEVER WAS. This documentation is necessary for law enforcement and the public to know that if/when male DBP tries to intercept messages, photos or phone calls to me he is stalking me and he is violating a mandatory protection order issued by a Magistrate to prevent DBP from coming near me or my dwelling or any place that he knows I would be such as my family of origin home etc. I removed the Pappas name from my name at the time of final divorce decree in Oct 2007 by order of Judge who signed final decree; and I resumed my born with name including my surname of Poole. I MOVED to Missouri on Oct 31, 2009 and into my own apt on Nov 3, 2009.

Diagnostic medical imaging is a known carcinogen written by Gloria Poole,RN, artist on 14 May 2015

I, Gloria Poole,RN, artist am adding in some quotes to cause people to know that all medical scans carry a risk of causing cancer. The more radiation a person is exposed too, the more likely that person will develop cancer. I am numbering for ease of reference. This is a huge topic and I am not attempting to cover it comprehensively. I am only trying to make public aware that it is so, and that there is much documentation about it.

1) From http://www.ncbi.nlm.nih.gov/pubmed/21123642 and then from http://www.ncbi.nlm.nih.gov/pmc/articles/PMC2996147/: "This review provides a practical overview of the excess cancer risks related to radiation from medical imaging...Radiation-induced risk is more controversial at doses between 10 and 100 mSv, the dose range relevant to medical imaging and in particular CT. A single CT of the abdomen may have a dose of around 10 mSv, and patients who undergo multiple CTs or a single multiphasic CT fall into this dose range. Nuclear cardiology examinations also typically fall in this dose range. Some investigators suggest that direct epidemiological data from atomic bomb survivors and nuclear industry workers indicate increased cancer risk in this dose range..."

2) "Although ionizing radiation has long been recognized as a potential carcinogen capable of introducing harm to patients, the long-term derived benefits have been accepted because of the diagnostic and therapeutic benefits derived from its use. With the increased utilization of high-dose ionizing radiation examinations (such as multidetector computed tomography), it is essential that the medical community create data-driven methodologies to quantify risk in objective terms, develop community-wide radiation standards and "best practice" guidelines (evidence-based medicine), develop new technologies and applications to proactively minimize radiation dose while maintaining quality, and create accountability measures for all pertinent stakeholders."http://www.ncbi.nlm.nih.gov/pubmed/19643383.

3)About Computer tomography [CT scans aka cat scans]: " First, it is important to understand the patterns of use and increasing application of CT, particularly multidetector CT. In addition, it is helpful to be familiar with measures of radiation pertinent to CT and the doses provided by this modality. This foundation then provides a context in which to discuss the issue of low-dose radiation and cancer risk as well as potential changes in CT practice guidelines and regulation. "http://www.ncbi.nlm.nih.gov/pubmed/17411538.

4) "This analysis shows that paediatric CT scans are adjusted according to the patient's age in most of the routine CT examinations. This indicates increased awareness regarding radiation risks associated with CT. However, high tube current settings are still used in younger patient groups, thus, optimisation of paediatric CT protocols and implementation of current guidelines, such as age-and weight-based scanning, should be recommended in daily practice."http://www.ncbi.nlm.nih.gov/pubmed/22970059.

5) "Computed tomography (CT) has been recognised as the most widely used imaging technique in both adults and children, owing to technological developments, especially with the recent innovations in multislice CT. This has resulted in an increase in the use of CT examinations in children younger than 15 years of age in developed countries. The increasing use of paediatric CT in clinical practice has raised concerns regarding the potential risk of radiation-induced malignancy. This is because CT examinations deliver a much higher radiation dose than conventional radiographic techniques. Children are more sensitive to radiation exposure than adults and have a longer time ahead of them to manifest radiation-induced effects and injuries. Therefore, it is of paramount importance to reduce or minimise the radiation dose to children when choosing CT as the major imaging modality for diagnostic purposes."http://www.ncbi.nlm.nih.gov/pubmed/20658103.

6) The topic of how to treat a person who was over-radiated has 1155 results by searching :http://www.ncbi.nlm.nih.gov/pubmed/?term=how+to+treat+over+radiation+with+scans includes DCT scans, MRIs, CT scans, SRT scans, FDG-PET scans .

If this causes people to question those annual exposures for so-called diagnostic tests that involve xrays or radiation [including scans] then it has accomplished the goal.

Cell phones are a Class 2 carcinogen

Quote: "In 2011, the International Agency for Research on Cancer (IARC), an arm of the World Health Organization (WHO), declared cell phones a Class B Carcinogen, meaning a “possible cancer-causing agent,” based on the available research."..."This places cell phones in the same category as diesel engine exhaust, some pesticides, and some heavy metals. The expert panel ruled that there was “some evidence” that regular cell phone use increased the risk of two types of tumors – brain tumors (gliomas) and acoustic neuromas."... "One mechanism of harm, published in 2010,5 explains how electromagnetic fields damage your cells and DNA by inducing a cellular stress response. The research was conducted by Dr. Martin Blank,6 PhD, a former Associate Professor at Columbia University in the department of physiology and cellular biophysics, and past president of the Bioelectromagnetics Society." And "A review of 11 long-term epidemiologic studies published in the journal Surgical Neurology7 in 2009 revealed that using a cell phone for 10 years or longer approximately doubles your risk of being diagnosed with a brain tumor on the same side of the head where the cell phone is typically held. "

Please read article at http://articles.mercola.com/sites/articles/archive/2015/01/06/cell-phone-use-brain-cancer-risk.aspx especially the 'common sense guidelines " because there are ways to minimize your exposure.

The National Institute of Health's opinion on EMFs: http://www.nlm.nih.gov/medlineplus/electromagneticfields.html

Also : http://www.niehs.nih.gov/health/topics/agents/emf/

From http://www.ncbi.nlm.nih.gov/pubmed/25738972 is "Mobile phone radiation causes brain tumors and should be classified as a probable human carcinogen (2A) (Review)." with quote :"We conclude that radiofrequency fields should be classified as a Group 2A ̔probable̓ human carcinogen under the criteria used by the International Agency for Research on Cancer (Lyon, France). Additional data should be gathered on exposures to mobile and cordless phones, other WTDs, mobile phone base stations and Wi‑Fi routers to evaluate their impact on public health."

I looked at these summaries of research and also saw that more research is pending, and what was especially notable in bad way is the increase of glioma attributed to use of cell phones.

This is primarily a #publichealth blog to educate the public on issues that affect the "masses" of the general population such as epidemics, airborne diseases, preventable disease including cancers, and preventable systemic diseases. I also post art I make to it rarely as a way of varying it some and because I have the artist label in the url address. But it is the only blog I post public health information too, of a comprehensive nature to give the public a good knowledge of an issue. I am a Registered Nurse with a license in Missouri; and worked in nursing in the state of Georgia and other states for 26 years in a variety of positions. In the course of that career, I got to see "the big picture" in how acute care facilities respond to events including natural disasters, and also how governments respond to them. I research to add to this blog but not a s an NIH scientist would. My words are written for people who are not trained in medicine or scientific research, generally speaking, so I try to make it very clear so any could understand the posts on the blog.

Also, on the topic of preventable disease is this article I read today:

Eating whole eggs is good for humans. I KNEW it: http://www.telegraph.co.uk/news/health/news/11514251/Four-eggs-a-week-can-reduce-risk-of-diabetes.html

Also, because the general public has become more educated on the issues, juries in medical malpractice and insurance lawsuits cases have been more knowledgeable also, and are more willing to punish hospitals, doctors and insurers who fail to follow standards of care. Read : Failure to treat medically by standards of care jury award of 28 million : http://www.latimes.com/business/la-fi-jury-awards-kaiser-cancer-patient-20150326-story.html.

I also create content and write on other blogs including blogs that are primarily for art I create. Because there is no intnetion to deeive, I notify the public of that fgrequently. You may see some of the art I created in past 24 hours on these blogs of mine:

Carbapenem-resistant Enterobacteriaceae, bacteria on endoscopes in news and is great threat globally, written by Gloria Poole,RN,artist of Missouri

Today I read an article in the L A Times about carbapenem-resistant Enterobacteriaceae, and thought to myself that I might research it some to add info for the public. So here is a summary of many documents and I have included links to the whole articles.
"CRE, which stands for carbapenem-resistant Enterobacteriaceae, are germs that have become too difficult to treat because they are resistant to most antibiotics. " and they are on endoscopes. http://www.latimes.com/business/la-fi-ucla-infections-20150219-story.html.

For more info  I accessed the National Institute of Health's search tool and found several articles on research into that bacteria. : http://www.ncbi.nlm.nih.gov/pmc/articles/PMC3947910/ discusses the various antibiotics that are effective against carbapenem-resistant Enterobacteriaceae,.
Quote : "The emergence of carbapenem resistance in Enterobacteriaceae is an important threat to global health. Reported outcomes of infections with carbapenem-resistant Enterobacteriaceae (CRE) are poor. Very few options remain for the treatment of these virulent organisms." And treatment : "Antibiotics which are currently in use to treat CRE infections include aminoglycosides, polymyxins, tigecycline, fosfomycin, and temocillin." And : n the USA, associated annual additional costs of infections caused by resistant organisms as compared to susceptible organisms are estimated between $21 billion and $34 billion (Spellberg et al., 2011). The Infectious Diseases Society of America (IDSA) recognizes antimicrobial resistance as “one of the greatest threats to human health worldwide” (Spellberg et al., 2011). In addition, antimicrobial resistance was the focus of the 2011 World Health Day of the World Health Organization (WHO). The impact of multidrug resistance (MDR) extends into all aspects of medicine and threatens the significant progress which has been made in transplantation, oncology, and surgery. Understandably, much emphasis has been appropriately placed on methicillin-resistant Staphylococcus aureus (MRSA). In recent years, several new treatment options have become available for MRSA. In addition, true vancomycin resistance remains fortunately rare. However, the threat of MDR in Gram-negative organisms has not led to a similar increase in novel therapeutics. The prevalence of carbapenem resistance in Enterobacteriaceae (CRE) isolated from clinical samples continues to increase globally (Prabaker and Weinstein, 2011; Rhomberg and Jones, 2009; van Duijn et al., 2011)."

And it seems there is link between CRE and Klebsiella Pneumoniae : "Over the past 10 years, dissemination of Klebsiella pneumoniae carbapenemase (KPC) has led to an increase in the prevalence of carbapenem-resistant Enterobacteriaceae (CRE) in the United States. Infections caused by CRE have limited treatment options and have been associated with high mortality rates." Entire article at http://www.ncbi.nlm.nih.gov/pubmed/?term=21653305.

I also researched the protocols that operating rooms are supposed to use in disinfecting endoscopes. Endoscopes cannot be sterilized in the same way as metal tools because they are lasers inside of flexible tools that drill down into body parts. And the disinfection regimen must be followed exactly ´VERY TIME. There are several websites on this topic but I prefer the CDC guidelines. That is probably because I worked as Registered Nurse in operating rooms in Atlanta, GA metro area and other states' also, but I actually visited the CDC in Atlanta GA [as an RN]  and know of their caliber.

Disinfecting endoscopes protocols: http://www.cdc.gov/hicpac/pdf/guidelines/Disinfection_Nov_2008.pdf
and also : http://www.sgna.org/Portals/0/sgna_stand_of_infection_control_0712_FINAL.pdf
and also : http://www.infectioncontroltoday.com/reports/2012/02/high-level-disinfection.aspx
Most helpful for medical people is : http://www.cdc.gov/hicpac/Disinfection_Sterilization/3_0disinfectEquipment.html with "reprocessing [re-use] of endoscopes as section to be read carefully by those who work in operating rooms with quote : "more healthcare–associated outbreaks have been linked to contaminated endoscopes than to any other medical device" and "Because of the types of body cavities they enter, flexible endoscopes acquire high levels of microbial contamination (bioburden) during each use ." And : "On the basis of these data, APIC 113, the Society of Gastroenterology Nurses and Associates (SGNA) 38, 114, 115, the ASGE 108, American College of Chest Physicians 12, and a multi-society guideline 116 recommend alternative contact conditions with 2% glutaraldehyde to achieve high-level disinfection (e.g., that equipment be immersed in 2% glutaraldehyde at 20oC for at least 20 minutes for high-level disinfection). Federal regulations are to follow the FDA-cleared label claim for high-level disinfectants. The FDA-cleared labels for high-level disinfection with >2% glutaraldehyde at 25oC range from 20-90 minutes, depending upon the product based on three tier testing which includes AOAC sporicidal tests, simulated use testing with mycobacterial and in-use testing. The studies supporting the efficacy of >2% glutaraldehyde for 20 minutes at 20ºC assume adequate cleaning prior to disinfection, whereas the FDA-cleared label claim incorporates an added margin of safety to accommodate possible lapses in cleaning practices. Facilities that have chosen to apply the 20 minute duration at 20ºC have done so based on the IA recommendation in the July 2003 SHEA position paper, "Multi-society Guideline for Reprocessing Flexible Gastrointestinal Endoscopes" 19, 57, 83, 94, 108, 111, 116-121.
Also, merely soaking the endoscopes in disinfectant is NOT enough. There are five steps involved in cleaning them for re-use and each step must be done, including the drying time.

"And : "In general, endoscope disinfection or sterilization with a liquid chemical sterilant involves five steps after leak testing:

1) Clean: mechanically clean internal and external surfaces, including brushing internal channels and flushing each internal channel with water and a detergent or enzymatic cleaners (leak testing is recommended for endoscopes before immersion).
2) Disinfect: immerse endoscope in high-level disinfectant (or chemical sterilant) and
3) perfuse (eliminates air pockets and ensures contact of the germicide with the internal channels) disinfectant into all accessible channels, such as the suction/biopsy channel and air/water channel and expose for a time recommended for specific products.
4) Rinse: rinse the endoscope and all channels with sterile water, filtered water (commonly used with AERs) or tap water (i.e., high-quality potable water that meets federal clean water standards at the point of use).
5) Dry: rinse the insertion tube and inner channels with alcohol, and dry with forced air after disinfection and before storage.
Store: store the endoscope in a way that prevents recontamination and promotes drying (e.g., hung vertically). Drying the endoscope (steps 3 and 4) is essential to greatly reduce the chance of recontamination of the endoscope by microorganisms that can be present in the rinse water 116, 156.  One study demonstrated that reprocessed endoscopes (i.e., air/water channel, suction/biopsy channel) generally were negative (100% after 24 hours;"

I am posting this information here in an effort to improve the global public health. Not all bona-fide ambulatory surgery places have guidelines for sterilizing invasive scopes. Metal surgical tools have their own set of sterilization guidelines.

I also tweet about this blog with hashtags : #publichealth, #gloriapoole; #ProlifeNurse; #gloria_poole but usually not all at the same time. You can follow me on twitter on sveral twitter accts of mine but the ones that often include medical facts or info are @ProlifeNurse; @personhood1.

Surgery on preborn helps save the baby humans: Update

I have written on this topic before in my effort to #savethebabyhumans. As a Registered Nurse with a long career I worked in a variety of paid positions. One of those was as Operating Room Circulator which is a defined role of assisting the surgeon and scrub team as the person who is not scrubbed up or "sterile" but who hands sterile supplies in a particular way to not contaminate them.  And another of those positions was as "first assistant" in the operating room, with duties to hold retractors, cut suture, be the second pair of hands for general surgeons.

The surgery on humans in the womb at the stage of development of "fetus" proves that those humans are HUMAN. The surgeons who do those surgeries eyeball and touch and operate on those tiniest humans in womb. I am including some quotes about a particular center that specializes on surgery on babies in the womb so you can read more about it and be persuaded that ending abortion as a so-called "right" is the thing to do and it is the current civil rights battle.

About the University of California at San Francisco's:
UCSF Fetal Treatment center is the Fetal Treatment Center  with quote: "of the most experienced institutions in fetal surgery and endoscopic fetal intervention (FETENDO fetal surgery) in the world. Our dedication to research, innovation, and clinical application has included the training of many dedicated physicians.

The Fetal Treatment Center combines the talents of specialists in pediatric surgery, genetics, obstetrics/perinatology, anesthesia, cardiology, radiology, nursing, and neonatal medicine. Our team of doctors, nurses, and social workers work together to coordinate all aspects of care from diagnosis to postoperative recovery and long-term follow-up."  And: "The Fetal Treatment Center is intricately connected with the UCSF Division of Pediatric Surgery at UCSF Benioff Children's Hospital which is dedicated to providing expert surgical care for fetuses, infants, children, and adolescents with congenital and acquired conditions".[end quote]

Also, Life News has compiled some medical quotes about the scientific / medical facts of the humanity of the babies in the womb; and you can read them at the link:

http://www.lifenews.com/2015/01/08/41-quotes-from-medical-textbooks-prove-human-life-begins-at-conception/

I am asking all people with a love of THE LIVING GOD to repent of having supported the cause of killing and to turn from that, and to P.U.S.H. * until abortion is outlawed in the United States.

For those women who do not choose life, you should read this article I wrote:

Also, I am adding in some photos and art I created to clearly identify this blog as one of mine for several reasons including copyright of my art and photos and words that I create; and for the sake of law enforcement to clearly identify me to prevent I D thieves from stealing my name or art, or anything else of mine. Also, fyi to @FBI and @Google: I received the fake Google login today to this account. The message in a woman's voice told me not to share my code with anybody and that Google "would never ask for it" but of course, Google asks for it! I set up the 2 step authorization on this account trying to prevent cyber-criminals from breaking into it. So, the code that is called to me is the login code. I know the fake code is coming from area code 202 in Washington DC because of caller ID; and  that the fake code is some sort of trick to allow criminals [probably the same ones who try to steal my T-mobile phone numbers] into my account. FYI.
This is my signature I signed in digital ink with an electronic "brush" to create a personal logo for 2015. I also created one in paint & posted it to other blogs of mine.

This is my photo of my left hand holding one of my Southern Baptist cross necklaces that do not have any graven images on it. The Southern Baptist/ Great Commission faith does not worship images/ idols/ icons of any body; nor does it bow to idols or statues.  I am Southern Baptist, born again Christian single woman and I worship THE LVING LORD only . You can read some of my reasons for that in Matthew 18:3-4 and also Mark 16: 15-18, King James Holy bible.

This is one of the photos I photographed in Dec 2014 on walkabout;  of the U S flag that flies near my apartment. I post this on web sites and blogs in various photos to remind all that I am a U S citizen with First Amendment liberties to publish my religious beliefs, words, ideas, goals, actions, photos, art I create and to worship GOD; and to protest the governments without interference by governments. Google the First Amendment to read the precise words of it since this paragraph  is my summary of the rights it conveys to U S citizens. I am a U S citizen by birth and have all civil liberties of the U S Constitution

Medical lesson on fevers, written by Gloria Poole, RN, artist of Missouri and Georgia

About fevers that develop suddenly
written by Gloria Poole, Registered Nurse, artist of Missouri and Georgia

This "lesson" for the public's health is about the diagnosis and treatment of a sudden fever, and is intended as a mere guideline for anyone anywhere who develops a fever suddenly or has a family member who does; or who is expected to care for a person with a fever.  Pay attention to what other symptoms the person with a fever develops. Does shaking of body or arms and legs occur  called "chills" ? Is there a general overall feeling of not being well, called "general malaise"?  Did the person with fever lose the appetite for food? Does the person have a headache? Does the person with fever have spots or rash on face or body? Can the person swallow normally? Can the person keep down liquids? Has the person with fever lost weight? When did the fever begin? Is the fever worse at night? Does the person with a fever also have a cough? If so,, does the coughing occur in spasms? Does the coughing cause the person to spit out mucus? Are the pupils of the eyes reddened? Does the person's nose run ? Does the person have vomiting or diarrhea? Does the person have bleeding from eyes, nose, ears, rectum?

In a normal healthy person, the normal temperature when taken by mouth varies from 96.5degrees fahrenheit [35.8  degrees Centigrade] to 99.0 degrees fahrenheit [37.2  degrees Centigrade] and usually the temperature is lowest between the hours of 2 -4 am, and highest between 6 -10pm. In very hot weather or after running, the body temperature may be 0.5 to 1.0 degrees  fahrenheit higher [03. to 0.6 degrees centigrade] temporarily. Most pharmacies sell some sort of thermometer for home use either the glass bulb sort that has mercury in it that is very durable but has to be disinfected between patients; or electronic ones which are much more expensive, have to be recalibrated at every use, and have to be replaced often.

Infection somewhere in the body is the most likely cause of any fever. Some infections cause other symptoms within a day or two. Some fevers can persist for 3 weeks without developing other symptoms. Most common infections are viral infections including infectious mononucleosis, viral hepatitis, varicella [chicken-pox], rubeola, [measles], rubella [German measles], human immunodeficiency virus {HIV], and rickettsial infections such as Rocky Mountain spotted fever, caused by tick bites, and Q fever caused by airborne rickettsiae in dust caused by contaminated animals or by direct contact with infected animals; Lyme disease which is a spirochetal infection caused by deer ticks; and a variety of bacterial infections including infections of the pleura, biliary tract, retroperitoneum, kidney, liver, spleen; and several acute [creating a crisis] bacterial infections including acute bacterial endocarditis [bacteria affecting the outer sac of the heart] and Salmonella bacteremia [food poisoning] , and Leptospirosis caused by contact with domestic or wild animals' urine in moist soil or water.

Also, if the person with the fever has traveled to other countries that he or she does not normally live in, the diseases prevalent in those countries must be taken into consideration when arriving at a diagnosis. Some of those diseases that cause fever are malaria, dengue fever, scrub typhus, tuberculosis, ebola,  plaque caused by rodents, typhoid, tetanus.

The incubation period varies for different fevers. Incubation period means the time it takes for symptoms to develop after the person was exposed to the virus or bacteria. The incubation period for chicken-pox is 10-21 days; for German measles it is 14-21 days, for ebola it is up to 21 days after exposure, for Leptospirosis it is 4-19 days, for measles it i 9 to 11 days, for Q fever it is 14- 26 days, for Rocky Mountain Spotted fever it is 3 to 14 days.

Hypersensitivity to prescribed medicines can cause fever also, but digitalis preparations and insulin almost never are the cause of fever. Medicines that frequently cause fever because of hypersensitivity of the patient to them are: Allopurinol, Amphotericin B, antihistamines, Atropine, barbiturates, Bleomycin, Captopril, cephalosporins, Clofibrate, Ethambutol, Heparin, Hydralazine, Ibuprofen. iodides, Isoniazid, Methyldopa, Nifedipine, Nitrofurantoin, p-Aminosalicyclic acid, Penicillinamine, Penicillins, Phenolphthalein, Phenytoin, Procainamide, Propylthiouracil, Pyrazinamide, Quniidine, Salicylates, Streptomycin. Sulindac, Sulfonamides. Fever may begin [promptly with the first dose of these drugs or occur weeks later, but a person who has taken any of these for longer than 3 months' time,  is not having fever because of these drugs. A drug-induced fever may be low grade or rise to 104 degrees fahrenheit [40 degrees centigrade]. Chills do not commonly occur with drug-induced fevers. But itching, rash, eosinphilia occur in many cases. In a few cases drug induced fevers progress to serum sickness [rash, swollen lymph nodes, arthritis, nephritis, swelling . Some drugs can cause a lupus-like syndrome of fever, arthralgias, and positive anti-nuclear antibody [ANA] titers, If removing the drug causes the fever to go away, that is a definitive establishment of cause and effect. If organ damage occurred with the administration of any drug, the drug should not be taken again. Administration-related fever may occur with any medicine give by intravenous method. Some medicines cause leukopenia and fever.

Other causes of fever are vascular occlusion events such as deep vein thrombophlebitis, minor pulmonary embolus, asymptomatic myocardial  infarction [ M.I., commonly called heart attack]. Also, fever may be the only symptom of acute autoimmune hemolytic anemia or hemolytic anemia due to glucose-6-phosphate dhydrogenase [G6PD] deficiency. Some vaccines cause fever.

The people who at most at risk of developing infections are those with preexisting conditions such as lymphoma, HIV, those receiving corticosteroids, those on immuno-suppressive drugs, rheumatic valvular disease, certain congenital heart diseases, multiple myeloma, splenectomy, sickle cell disease, granulocytopenia [less than 1,000 granulocytes/mm] , advanced cirrhosis, diabetes, alcoholism, and those who use illegal substances .

Fevers lasting more than 3 weeks are usually chronic conditions such as tuberculosis, subacute bacterial endocarditis, [SBE, chronic osteomyelitis, cytomegalovirus infections, intra-abdominal abscesses, urinary tract infections, HIV, systemic lupus erythematosus, temporal arthritis, certain neoplasms especially lymphoma, acute leukemia, hypernephroma, hepatoma, pancreatic carcinoma. carcinoma of lung, bone malignancies, granulomatous hepatitis, hyperthyroidism, drug fever, inflammatory bowel disease, sarcoidosis, thyroiditis, recurrent pulmonary emboli. If fever has continued longer than 3 weeks in spite of diagnostic studies, these diseases must be considered as the possible cause of it.

To diagnose fever, start with taking a  comprehensive history from the patient. Ask the questions in paragraph one and include other questions about possible exposure to other people who have symptoms of any disease? Any travel abroad? To where, when, for how long? Are other people in the same household sick or have a fever? Does the person with a fever have any of the diseases listed in the above paragraph? Does the person with fever take any of the drugs or illegal substances listed in the paragraph about hypersensitivity ?  After the history is written see if any of it suggests a path to take to diagnose? if not, start the diagnostic work-up  to include: a chest X-ray of both lateral and posterior-anterior views, a complete blood count with differential, erythrocyte sedimentation rate, liver function tests, at least two blood cultures [aerobic and anaerobic], urinalysis, quantitative urine culture if urinalysis reveals pyuria or bacteriuria . If symptoms are severe, the patient may be hospitalized and treated with prompt anti-microbial drugs to prevent sepsis.  A physical examination should be done intermittently as long as fever persists to see if organs are being affected; and should include examination of the eyes with opthalmoscope, palpation for an enlarged or tender temporal artery, listen for a new or changing heart murmur, palpate for an enlarged  or tender thyroid gland, listen for pericardial, pleural, or hepatic friction rubs, palpate the liver, spleen, abdomen, for swelling or tenderness, examine the rectum [and prostate in men], feel for enlarged or tender lymph nodes, look at mucus membranes of mouth, eyes; look at nail beds for color and any splinter hemorrhages. Give the person with fever a PPD [Purified protein derivative test] for tuberculosis  unless there is a documented history of a positive PPD or of tuberculosis. Examine blood smears if the person has traveled to countries where malaria exists in past six months. Evaluate medicines patient takes.  If any are on the list of medicines and are not essential, discontinue them. If they are essential, change the medicine to something not on the list above that are known to cause fever.

Have patient take their own temperate twice a day for at least  two weeks and chart it so you can see if a pattern emerges? If fever persists, continue diagnostic work-up with skin tests, electrocardiogram, serum calcium level, serum titers of antistreptolysin O, rheumatoid factor, antinuclear antibody, HIV antibody, and a Monospot test and repeat the complete blood count with differential. It is important at this point to have an independent observer/ care giver to test the temperature and record it for another week. A fever of unknown origin lasting more than 3 weeks probably should be evaluated in an acute care hospital. Fevers that have other symptoms especially causing hemorrhaging or vomiting and diarrhea need to be hospitalized as quickly as possible.

It is important to note that not all fevers have to be treated. Some will run their course and the patient will get well. The use of antipyretics may not be needed but if so, aspirin is a very good anti-pyretic but may cause the patient to sweat and have shaking chills. That response can be diminished by giving the patient aspirin in dose of .03 to 0.g grams regularly at 3 to 4 hour intervals. If patient is allergic to aspirin, or at risk of bleeding,  acetaminophen may be given.  Unless the patient is acutely ill, with involvement of organs, it is considered best to hold off on treatment with antibiotics until you know what you are treating.

To prevent contamination of other persons the care taker and or medical doctor must use good handwashing technique including scrubbing under fingernails with a strong disinfectant and a nail brush and washing any part of their body that came into contact with infected person. If the virus or bacteria is airborne [spread by droplets through the breathing in and out of the infected person] then masks and covering head to toe must be  apart of the regimen of care. See the links in previous posts on how to do that.

I referred to medical textbook, Principles of Ambulatory Medicine, edited by L. Randol Barker, MD; John R Burton,MD,;and Phillip Zieve, MD; chapter 26 to summarize and put into my own words so that average persons could understand this.

13 Dec  2014: Copyright notice: this blog and all content on it is created by me and owned by me  Gloria Poole residing in Missouri but born in the state of Georgia, and it is covered by U S copyright law. I, Gloria Poole, own all rights to this blog and to all content on it, all words, all photos, and all art represented by photos that I photographed. I have the actual, tangible art I created and photographed.  This blog and or its individual posts may not be transferred to anyone anywhere, nor have domains forwarded to it that do not belong to me, nor be saved to disk, nor downloaded, nor printed at remote,  nor copied, nor photo-copied [screen-captured] , nor in any way with any method be reproduced. I, Gloria Poole, own all rights to all words, art and photos I create  with any method of technique or medium on any surface anywhere any location any reason or no reason, whether or not I photograph it on any camera, phone, etc, and whether or not I publicly display it anywhere. I have never signed a blanket waiver of my copyrights to art I have created and do not have an artist's rep [agent], nor literary agent to represent me, and never did.  I drew,  painted, signed, photographed and uploaded all art on this blog and on any blog of mine.

For the record and to establish provenance of art I have already created also: the art I have created and signed and posted to this blog of mine is part of an on-going series of art I created since year 1991 beginning in Atlanta, GA that "blossomed"  in year 2005 in  Aurora, Colorado when I lived there for one year [May 2006-May 2007], and art began to really become an integral part of my efforts to save the baby humans from premeditated destruction. In 2005, I began to realize I could use the art to illustrate my talking points in some cases, and to build a secondary "career' for me as artist.  In the year 2006 when I had endured and lived to tell about it a violent crash down a flight of stairs and had trauma, fractures, concussion, because my then-husband DBP tried to kill me to 'make it look like an accident" and he tried to twist off my broken leg, I had immobility for months except on crutches and walking boot after surgery to repair fractures and implant metal. As I sat with leg propped up I began to draw daily and paint to occupy the time and my mind. I also began to post art to the web on my blogs and websites, and also photographs I had photographed. I began a series of art then that is named "ethnic series by Gloria" [me] to attempt to draw and paint an art work of every ethnic group tribe, language, culture in the world and I created eight initial oil paintings in the first batch and posted them to the web on domains of mine.  I moved from Aurora to zip code 80203 for 2 and 1/2 years and then moved to Missouri on Oct 31, 2009 with the art I had in my possession at that time. I paint some of the sketches I created  in do-overs into oils on canvas and those are very visible on the web. I put many of them on my mini exhibit of art at https://mini-exhibition.blogspot.com, and also on my Picasa albums at https://picasaweb.google.com/gloriapoole.

I, Gloria Poole, am a white, Southern Baptist Christian, brown-haired woman, single-again, twice-divorced,natural mother of only two children who are grown daughters named Jennifer and Leigh, and also a republican, personhood promoter, prolife activist-blogger, photographer, artist in all mediums, poet, author, illustrator, cartoonist, writer, University of Georgia alumna, U S citizen born in the state of Georgia [but I lived in several places including New York, UK briefly, Nebraska, South Carolina, Colorado, Virginia, North Carolina], former TV producer, tweeter, Registered Nurse licensed in Missouri [but before that in Georgia, UK, other states], owner/writer/blogger/photographer/illustrator for words that WORK, and also for Tapestry of LIFE,and also for Life Media & Publishing in years 2004-2011; and a just-for-fun-photographer, and citizen journo . Gloria Poole is my real, born with legal name and I resumed my full maiden name including my surname of Poole legally by Court order at the time of divorce from male DBP in Colorado in Oct, 2007 at Centennial, Colorado; and I also removed the Pappas name from my name forevermore at the time of final decree in Oct 2007 . I was glad to end that four year horrible marriage of multiple trauma and injuries to me caused by male DBP, [which I testified about in Court on several occasions ].  That second divorce is public record in the state of Colorado, and my first divorce is public record in the state of Georgia. I have created art regularly since 1991, and I had formal training in drawing and in mixing colors /paint and painting in oils and I post much of the art I create on blogs of mine. You can see some of that art on one of my name blogs at https://gloriapoole.blogspot.com and see the about me page of that blog for the list of art blogs I own and post art too, that are all different. .
